In this cohort study of 475 surgical patients with preoperative wearable data, lower baseline daily step counts were associated with higher odds of postoperative complications. Patients averaging fewer than 7,500 steps per day had approximately twofold increased adjusted odds of complications, suggesting step count as a marker of preoperative fitness and risk.
