Over 177,000 drug–drug interaction (DDI) reports were identified, with nearly half resulting in hospitalization and more than 8% in death.

Adults aged 65–85 and patients with multiple comorbidities faced the highest incidence of severe outcomes.
Routine medication reconciliation and interspecialty communication remain critical to prevent avoidable adverse events from pharmacologic interactions.
