The 10-year BOSS trial demonstrated that biannual endoscopy surveillance does not improve overall or cancer-specific survival outcomes in low-risk patients with Barrett's esophagus, suggesting that at-need procedures may be sufficient. Conducted across 109 UK centers and involving 3,453 participants, the trial compared traditional surveillance with endoscopy performed only upon symptom onset. Findings indicated no significant differences in mortality rates or cancer diagnosis stages between the two groups.
