In this randomized clinical trial of patients at nutritional risk undergoing pancreatoduodenectomy, early supplemental enteral nutrition in addition to oral intake significantly reduced the overall burden of postoperative complications compared with oral nutrition alone, as measured by the Comprehensive Complication Index at 90 days. While overall morbidity rates and specific surgical complications were similar between groups, enteral nutrition was associated with fewer infectious and pulmonary complications and was generally well tolerated.
