mRNA Boosters Drive Antibody Evolution in Immunosuppressed Patients

Successive mRNA vaccinations enhanced cross-variant antibody evolution in immunosuppressed patients, though responses varied by treatment type, according to a new study.

The evolution of the SARS-CoV-2 antibody repertoire following successive mRNA vaccinations in immunosuppressed patients was investigated by analyzing 878 participants, leading to the observation of increasing anti-BA.1 RBD reactivity over time. Affinity maturation and the impact of different immunosuppressive treatments on cross-variant antibody evolution were explored. Patients receiving anti-CD20 therapy showed minimal to no antibody evolution, indicating that repeated mRNA vaccination may not elicit broad cross-variant protection in this patient population. Prior SARS-CoV-2 infections accelerated the emergence of BA.1 RBD reactivity following the first two vaccinations but did not significantly alter the response compared with vaccination alone after three doses.
