The American Gastroenterological Association (AGA) announced new evidence-based guidelines on the use of fecal microbiota transplants in most patients with recurrent Clostridioides difficile infections; they were published by Peery et al in Gastroenterology. Experts noted that transplants are safe and effective; they involve the transfer of stool from a donor with a healthy gut microbiome to the colons of patients with recurrent C diff infections via colonoscopy. The AGA recommended that patients with recurrent C diff infections at high risk of recurrence undergo standard antibiotic therapy followed by fecal microbiota transplant; however, they stressed that patients with inflammatory bowel disease or irritable bowel syndrome and those who are severely immunocompromised—including patients who are neutropenic or have received bone marrow transplants—should not undergo fecal microbiota transplants. They also recommended that some patients hospitalized with severe infections undergo conventional fecal microbiota transplants if no improvement is observed with standard antibiotic therapy. The AGA hopes its new guidelines can help treat the nearly 100,000 recurrent C diff infections reported per year.
