Brief Prehabilitation Reduced Complications Prior to Gastrectomy

In this multicenter randomized clinical trial of 347 older patients with frailty undergoing radical gastrectomy, a supervised, home-based multimodal prehabilitation program combined with Enhanced Recovery After Surgery care significantly reduced postoperative complications (17% vs 29%) and improved functional capacity, including increased 6-minute walk distance before surgery. Prehabilitation was also associated with faster postoperative recovery, shorter intensive care unit and hospital stays, and improved physiological resilience, supporting its role as an effective perioperative strategy in this high-risk population.
