This case report describes a 66-year-old man with poorly controlled diabetes who developed urinary bladder obstruction due to multiple Candida fungal balls, presenting with urinary symptoms and hematuria. CT imaging revealed characteristic mixed-attenuation intravesical nodules with gas and associated emphysematous cystitis, which can mimic other conditions but are suggestive in high-risk patients. Despite antifungal therapy and improved glycemic control, the fungal masses progressed, causing near-complete obstruction, highlighting that medical therapy alone is often insufficient and procedural removal is typically required.
