In a meta-analysis of 285 studies including more than 337,000 patients, psychotic symptoms occurred in approximately 53% of patients with bipolar disorder, while comorbid schizophrenia was present in about 8%, highlighting substantial but differentiated overlap between the conditions. Psychosis was more frequent in bipolar I vs bipolar II disorder (63% vs 17%) and during manic vs depressive episodes (~70% vs ~43%), with delusions the most common symptom, although findings were limited by high heterogeneity across studies.
