In a nine-week-old infant with progressive constipation, feeding difficulty, and symmetric descending flaccid paralysis, bedside nerve conduction studies supported an early clinical diagnosis of infant botulism, and equine-derived heptavalent botulinum antitoxin was administered on hospital day four prior to laboratory confirmation of Clostridium botulinum type B. Rapid recognition, multidisciplinary care, and subsequent administration of human botulism immune globulin IV were followed by full neurologic recovery and discharge home by hospital day 17
