A retrospective study of 13 patients with cutaneous epithelioid or pleomorphic rhabdomyosarcoma found that 10 cases were best reclassified as transdifferentiated melanoma after comprehensive molecular and epigenetic analysis. Despite uniform expression of skeletal muscle markers and consistent negativity for standard melanoma markers, the majority of tumors demonstrated UV mutational signatures, elevated tumor mutation burden, TERT promoter mutations, and DNA methylation profiles clustering with desmoplastic melanoma — findings incompatible with primary rhabdomyosarcoma.
