This case report describes a previously healthy 37-year-old man who developed rapidly progressive necrotizing fasciitis (Fournier’s gangrene) of the scrotum and right lower extremity following minor bicycle-related abrasions. Despite initial misdiagnoses and delayed treatment, the patient deteriorated to septic shock and multi-organ failure. Diagnosis was ultimately confirmed through imaging, a high LRINEC score, and identification of Streptococcus pyogenes. Prompt multidisciplinary intervention—including emergency fasciotomy, targeted high-dose penicillin therapy, and intensive supportive care—resulted in full recovery without long-term complications, highlighting the importance of early recognition and coordinated management.
