In this randomized clinical trial of 850 patients with gram-negative bloodstream infections, rapid antimicrobial susceptibility testing directly from positive blood cultures was not superior to standard testing in improving 30-day desirability of outcomes. Although rapid testing shortened time to antibiotic escalation or de-escalation, it did not improve time to effective therapy or other clinical outcomes.
