New XLH Guidelines Shift Primary Therapy to Burosumab

The updated clinical practice recommendations, published in Nature Reviews Nephrology, have established burosumab as the primary therapy for pediatric patients with X-linked hypophosphatemia (XLH) presenting with various symptoms. The guidelines also provide revised guidance on the diagnosis and management of XLH across different age groups. Burosumab has shown improved safety and efficacy compared to conventional treatments, with reduced gastrointestinal issues and lower risk of hyperparathyroidism. The guidelines recommend biweekly dosing of 0.8 mg/kg in pediatric patients and monthly doses of 1.0 mg/kg in adult patients, with a maximum of 90 mg. Monitoring includes serum phosphate levels but not fibroblast growth factor 23 (FGF23) levels. Long-term studies have reported benefits of reduced dental abscesses and improved quality of life in pediatric patients treated with burosumab. However, further research is recommended to develop clinical scores for evaluating treatment efficacy, optimize surgical timing, and assess long-term outcomes.
