Study Examines SGLT2 Inhibitor Use Before Surgery and Diabetic Ketoacidosis Risk

Researchers found no significant association between preoperative sodium-glucose cotransporter 2 inhibitor use and postoperative diabetic ketoacidosis in patients undergoing emergency surgery.

A retrospective cohort study assessed whether preoperative use of sodium-glucose cotransporter 2 (SGLT2) inhibitors was associated with an increased risk of postoperative diabetic ketoacidosis (DKA) in patients undergoing emergency surgery. The study, published in JAMA Surgery, involved 34,671 patients with type 2 diabetes (T2D) who underwent 13 emergency surgical procedures. Results indicated no statistically significant difference in the risk of postoperative DKA between the SGLT2 inhibitor group and the non-SGLT2 inhibitor group. These findings suggest that current guidelines on preoperative SGLT2 inhibitor discontinuation may warrant reconsideration to avoid perioperative hyperglycemia and surgical delays.
