A network meta-analysis of 11 randomized controlled trials involving 2,042 patients with severe community-acquired pneumonia found that hydrocortisone was the only corticosteroid associated with lower all-cause mortality and reduced need for mechanical ventilation compared with placebo. Dexamethasone, methylprednisolone, and prednisolone were not associated with statistically significant mortality benefits. Low-to-moderate corticosteroid doses and short-course therapy also were associated with improved outcomes, although evidence certainty was rated very low to low because of heterogeneity, imprecision, and reliance on indirect comparisons.
