1. Selective parathyroid hormone-guided supplementation was not superior to routine calcium and calcitriol after total thyroidectomy.
2. Symptomatic hypocalcemia rates were similar (7.8% selective vs 11.1% routine).
3. Biochemical hypocalcemia also showed no significant difference between strategies.
4. Selective management required less calcium supplementation overall.
5. Both strategies are reasonable options depending on clinical setting and resources.
