In a 15-year-old girl with persistent right anterior knee pain and a varus-hyperextension gait pattern resembling knee subluxation, imaging showed excessive femoral anteversion and external tibial torsion that were not detected on plain radiographs or magnetic resonance imaging. Rotational malalignment correction with femoral and tibial torsional osteotomies normalized gait mechanics and improved clinical outcomes, highlighting the importance of assessing torsional deformities in patients with unexplained gait dysfunction and knee pain.
