In this national retrospective cohort study of 22,011,131 live births and 12,993 pregnancy-associated deaths from 2018 to 2023, states with complete or 6-week abortion bans did not demonstrate statistically significant increases in pregnancy-associated, pregnancy-related, maternal, or nonobstetric mortality compared with nonban states in early post-Dobbs analyses. Although overall mortality declined across all groups, decreases were smaller in ban states and descriptive increases were observed among some racial and ethnic groups, highlighting the need for continued surveillance given the short follow-up and wide confidence intervals.
