This case report describes a 46-year-old woman with refractory erythromelalgia, a rare neurovascular pain disorder, who experienced rapid and sustained symptom remission following initiation of semaglutide for obesity, independent of significant early weight loss. Findings suggest a potential mechanistic role of glucagon-like peptide-1 receptor agonists in modulating neurovascular, inflammatory, and endothelial pathways, although causality and generalizability remain unproven.
