1. Oral ivermectin did not demonstrate noninferiority to 5% permethrin for scabies cure at day 28.
2. Permethrin achieved higher cluster-level cure rates (88.5% vs 71.8%).
3. Superiority of permethrin was consistent across index cases and household members.
4. Adverse event rates were low and similar between groups.
5. Permethrin should be preferred first-line when topical therapy is feasible.
