Broken Arrow oral surgeon Rocky Cullens has agreed to pay $532,055.76 to resolve civil allegations under the Oklahoma Medicaid False Claims Act, according to a press release from the Oklahoma Attorney General’s Office.
The investigation, prompted by a referral from the Oklahoma Health Care Authority, found that Cullens—operator of Stonewood Hills Oral Maxillofacial Surgery Inc.—allegedly submitted false claims to SoonerCare between January 1, 2021, and July 31, 2024.
Alleged violations included double billing, upcoding patient visits, billing excessive units of general anesthesia, and submitting claims for tobacco cessation counseling in non-tobacco users. The settlement includes repayment of $354,703.84 in SoonerCare reimbursements and $177,351.92 in penalties. The claims were resolved with no admission of liability and no formal determination of wrongdoing.
The case was handled by Assistant Attorney General Jamie Bloyd, Agent Lauren Jewell, and Nurse Analyst Laurie Hudson in the Attorney General’s Medicaid Fraud Control Unit. The unit receives 75 percent of its funding—$4,527,129.64 for FY 2025—from the U.S. Department of Health and Human Services, with the remaining $1,509,367.95 from the State of Oklahoma.
“I take seriously allegations of false claims and attempts to defraud taxpayers,” stated Oklahoma Attorney General Gentner Drummond. “I’m pleased our investigation led to a resolution that rightfully returns funds to the State.”
