In a cohort study of adults with hypertension, prediabetes was associated with a higher risk of incident heart failure, particularly among those with evidence of subclinical myocardial injury or stress as measured by high-sensitivity cardiac troponin and N-terminal pro–B-type natriuretic peptide. The findings suggest that integrating glycemic status with cardiac biomarker assessment may improve heart failure risk stratification in hypertensive patients before progression to overt diabetes or symptomatic cardiovascular disease.
