A randomized clinical trial compared intradetrusor onabotulinumtoxinA injection with midurethral sling surgery for females with mixed urinary incontinence. Both treatments showed improvement in symptoms at 6 months, challenging the historical approach of treating the urgency component first. The study findings demonstrated no significant difference in symptom improvement between the two groups but found greater improvement in stress incontinence symptoms with midurethral sling surgery. The study population primarily consisted of White women with a mean age of 59 years.
