Researchers evaluated whether prophylactic oral amoxicillin with clavulanic acid reduces wound complications after episiotomy or second-degree perineal tear. The REPAIR trial enrolled 442 postpartum patients who received either three doses of amoxicillin–clavulanic acid or placebo within six hours of delivery, with follow-up at 4 to 14 days. Although overall wound complication rates were similar between groups, clinically relevant complications—including larger dehiscence, significant pain, and infections requiring follow-up—occurred in 9% of patients receiving antibiotics compared with seventeen percent receiving placebo. The antibiotic group also had fewer unplanned consultations and additional antibiotic treatments.
