In this large retrospective cohort study of 123,762 women, artificial intelligence-based automated quantification of breast arterial calcification on screening mammography showed a strong, dose-dependent association with major adverse cardiovascular events and all-cause mortality, independent of traditional risk factors and the PREVENT risk score. Incorporating breast arterial calcification into risk models improved prognostic discrimination, suggesting that routine mammography may serve as an opportunistic tool for cardiovascular risk stratification and earlier preventive care in women.
