Predictive Rituximab Protocol Reduces Pemphigus Relapse Risk

French multicenter study demonstrates targeted rituximab maintenance therapy guided by clinical and serological markers cuts early pemphigus relapse rates from 17.6% to 2.6%

A French multicenter cohort study published in JAMA Dermatology found that using predictive factors to guide additional rituximab treatment significantly reduced early relapse rates in patients with pemphigus. The study evaluated 87 patients and identified three key predictors of relapse: a baseline PDAI score ≥ 45, DSG1 antibodies > 20 IU/mL, and/or DSG3 antibodies > 130 IU/mL at 3 months. Among the patients who achieved complete remission at 6 months, those with predictors who received additional rituximab had a relapse rate of 0%, compared to a historical relapse rate of 17.6% from previous studies. The study authors concluded that the findings validate the use of predictors of early relapse after an initial cycle of rituximab in patients with pemphigus.
